OUR PHILOSOPHY
Hospice is a specialized form of care that focuses on providing comfort, support, and dignity to individuals who have a life limiting illness. The primary goal of hospice care is to enhance the quality of life for patients and their families during this challenging time. Hospice care is provided by a team of healthcare professionals, caregivers, and volunteers who work together to address the physical, emotional, psychological, and spiritual needs of patients and their loved ones.

Hospice care can be provided in various settings, including home, skilled nursing facilities, assisted living facilities, board and care homes. Hospice services are typically covered by Medicare, Medi-Cal, and most private insurance plans.

Overall, hospice philosophy aims to create a peaceful and comfortable environment that allows individuals to spend their remaining time surrounded by loved ones, while receiving compassionate and comprehensive care that respects their individuality and wishes.

TEAM
We are proud to have a skilled and dedicated team of professionals who are passionate about providing the highest level of care. Each member of the team brings a unique set of skills and expertise to address various aspects of physical, emotional, and spiritual well-being, while also offering support and guidance to the patient's family members throughout their journey



PHYSICIAN
Oversees the medical care provided to patients. They work closely with the patient's primary care physician and other specialists to ensure that the patient's medical needs are met. The physician reviews care plans, provides guidance on pain and symptom management, and ensures that care aligns with the patient's wishes.
NURSE
Skilled registered nurses (RNs who specialize in end-of-life care. They provide direct care to patients, monitor symptoms, administer medications, and offer emotional support. Hospice nurses also educate patients and families about the patient's condition and provide guidance on caregiving.
SOCIAL WORKER
Social workers in hospice focus on the emotional and psychosocial well-being of patients and their families. They assess the patient's and family's needs, provide counseling and emotional support, and help with practical matters such as advance care planning, resources, and connecting with community services.



SPIRITUAL COUNSELOR
Offer spiritual and emotional support, tailored to the patient's belief system. They provide guidance, comfort, and assistance in addressing existential concerns, as well as support for grieving family members
AIDE
Provide hands-on care to patients, assisting with activities of daily living such as bathing, dressing, grooming, and mobility. They play a crucial role in maintaining the patient's comfort and dignity
VOLUNTEER
Trained volunteers contribute their time to provide companionship, respite care, and emotional support to patients and families. They may engage in activities such as reading, playing music, or simply being a comforting presence
SERVICES
Medications
Our team carefully manages all prescribed medications related to comfort and symptom control. We work with the doctor to order, deliver, and adjust medicines as needed to ease pain, breathing problems, nausea, anxiety, and other distressing symptoms. Our nurses also teach families how and when to give each medication so your loved one can remain as comfortable and safe as possible.
Equipment
Medical Equipment under hospice includes any equipment needed to keep the patient safe and comfortable, such as hospital beds, wheelchairs, walkers, oxygen equipment, bedside commodes, shower chairs, and similar items related to the medical conditions
Supplies
Everyday, mostly disposable items used to keep the patient safe, clean, and comfortable while managing their illness and symptoms. such items can include, wound care dressings, incontinence products, disposable gloves, catheters, and basic personal care items such as wipes, soap, and skin‑protective creams, all provided and replenished as needed as part of the hospice plan of care
BEREAVEMENT SUPPORT
Hospice care extends support to the family after the patient's passing through bereavement services, offering counseling, support groups, and resources to help loved ones cope with their loss.
